Case management involves the collaborative efforts to assess, plan, implement, coordinate, monitor, and evaluate the options and services required to meet a patient’s health and human service needs. The Commission for Case Manager Certification, the first and largest nationally accredited organization that certifies case managers, says case management is characterized by advocacy, communication, and resource management and promotes quality and cost-effective interventions and outcomes.
